 Protecting your private information is our priority. This Statement of Privacy applies to LindyGlow.com and Lindy Glow and governs data collection and usage. For the purposes of this Privacy Policy, unless otherwise noted, all references to Lindy Glow include LindyGlow.com. The Lindy Glow website is a Hormone Lifestyle Coaching site. By using the Lindy Glow website, you consent to the data practices described in this statement.

Collection of your Personal Information:
In order to better provide you with products and services offered on our Site, Lindy Glow may collect personally identifiable information, such as your:

  • First and Last Name

  • Mailing Address

  • E-mail Address

  • Phone Number

If you purchase Lindy Glow's products and services, we collect billing and credit card information. This information is used to complete the purchase transaction.

Lindy Glow may also collect anonymous demographic information, which is not unique to you, such as your:

  • Gender

  • Race

  • Religion

We do not collect any personal information about you unless you voluntarily provide it to us. CALIFORNIA SENATE BILL-577 WHAT IT MEANS FOR PATIENTS

California Senate Bill-577 (SB-577), which was signed by the governor in September 2002, has profound implications for the practice of alternative forms of healthcare in California. SB-577 enables alternative and complementary healthcare practitioners to provide and advertise their services legally. However, they must also comply with certain requirements specified within the bill.

What does SB-577 mean for you, the patient?

SB-577 GIVES YOU ACCESS TO ALTERNATIVE AND COMPLEMENTARY HEALTHCARE PRACTITIONERS.

You must be given information about the nature of treatment and the practitioner’s qualifications. Feel free to ask a practitioner any questions you might have about your treatment. Check to see if your practitioner has been certified by a professional membership society. In addition, tell your doctor about any alternative treatment(s) you are pursuing. You can also request that your licensed and unlicensed healthcare providers communicate with each other and work collaboratively to meet your healthcare needs.

SB-577 HELPS TO PROTECT YOU.

SB-577 requires unlicensed alternative healthcare practitioners to follow certain guidelines and restrictions.

Here are things that unlicensed alternative practitioners are NOT allowed to do:

  • Perform any form of surgery or any procedure that punctures your skin or harmfully invades your body.

  • Use X-ray radiation.

  • Prescribe prescription drugs, or recommend that you discontinue drugs that were prescribedby a licensed physician.

  • Set fractures.

  • Treat wounds with electrotherapy.

  • Put you at risk of great bodily harm, serious physical or mental illness or death.

  • Imply in any way that they are licensed physicians.In addition, an unlicensed alternative practitioner MUST DO the following things:

  • Provide you with a statement, written in plain language, that includes the following information: (1) that they are not a licensed physician and that their services are not licensed by the state; (2) a brief and clear description of the kind of services they provide and the reasoning behind it; and (3) a description of their education, training and experience.

  • Ask you to sign an acknowledgement that you received the above-written statement and provide you with a copy of it. They must also keep a copy of your signed acknowledgement for three years.

Informed Consent for Find Your Glow Hormone Lifestyle Coaching

California Senate Bill SB-577, effective January 1, 2003, enables complementary and alternative health practitioners to provide and advertise their services legally, according to certain rules of compliance specified in the legislation. Section 2053.6 of the California Business and Professions Code requires the following disclosures:

Disclosure to the client in plain, written language that:

  1. that the practitioner is not a licensed physician;

  2. that the treatment is a complementary or alternative healing modality and not licensed by the state;

  3. the nature of the service to be provided and the theory of treatment upon which the services are based; and 

  4. the practitioner‘s education and other training, experience, and other qualifications regarding the services to be provided.

I understand that Melinda Markulis is not a licensed physician in California and does not possess a license from the state of California. 

I also understand that Hormonal Life Coaching is a form of complementary and alternative medicine not licensed by the State of California. In practicing complementary and alternative medicine Melinda Markulis seeks to support rather than replace the care of your existing physician, therapist, or acupuncturist.

Melinda Markulis provides Life Coaching treatments and exercises designed to assist clients in maintaining physical, emotional, and spiritual well-being. 

Find Your Glow, Hormone Lifestyle Coaching, is a system of health care that recognizes the root causes of symptoms or disease, and treats the client as a whole person. Practiced as an adjunct to Western medicine, hormone lifestyle coaching addresses the infradian rhythm imbalances or blockages that contribute to illness. 

By using this site you understand the nature of Hormone Lifestyle Coaching and the above-referenced practitioner’s qualifications to practice Find Your Glow.

Terms and Conditions

Payments: A non-refundable Reservation Fee of $100 is required to hold your spot and may be made via credit card, Cash, Venmo or Paypal at time of booking. Deposit amount will be deducted from the total program cost. Payment must be made in full 24 hours in advance of the start date. There are payment plans. Refund and Return Policy

Reservation Fee is non-refundable. Once payment is made in full there are NO refunds. View Cancellation Policy for rescheduling instructions. If a Client misses more than 2 weeks her Find Your Glow program will be terminated with no refund available.

 
 
 
 

Cancellation Policy

 Cancellations: Please give a minimum of 24 hour notice for cancellations, in order to allow others the opportunity to schedule in your stead. Cancellations made fewer than 24 hours are at higher risk of not being able to reschedule for that week. Cancellations made fewer than 24 hours in advance and No-Shows will be charged the full session rate. Cancellations accepted online no fewer than 24 hours in advance. If you must cancel fewer than 24 hours in advance, please contact me directly.

Cancellation fees DO NOT APPLY. If you cancel your one on one call for the week you have the option of rescheduling within the following three days OR rolling over into the next week. Meaning if you are on Week 3 of Find Your Glow you can cancel and pick up the following week still on week 3. Find Your Glow allows one week to be canceled or missed.

All other appointments will have to be rescheduled for the existing week of the program. You are allowed only ONE week to cancel.

If Two weeks are canceled your Find Your Glow Program will be terminated. This is to insure your growth in the program. If you choose to continue the program at a different date of time, your program will start at the beginning.
